Emergency Plan

pdf file A Food Allergy Emergency Plan Form is a document which helps our food service staff identify students with special dietary needs. Posted in the kitchen or office area, and out of public view, this form helps to ensure our staff is aware of those students who have allergies, intolerances or other special dietary needs.

It also provides us with additional information on treatment, symptoms and your emergency contacts. It quite simply provides an additional layer of protection. Completed forms should be submitted to the Unit Manager or Unit Chef in early September. The completion of this form is entirely optional.

 
 

Getting adequate intakes of folate is especially important for women who may become pregnant. Women of childbearing age or planning to become pregnant should eat plenty of folate-rich foods and take a daily multivitamin or mineral supplement that contains 0.4 mg (400 ug) of folic acid.
